All RCRA Large Quantity Generators (>1000 Kg / Mo) and permitted Treater, Storer, or Disposer (TSD) facilities must complete the 2011 Annual Hazardous Waste Report according to LAC 33:V.  Small Quantity Generators and Conditionally Exempt Small Quantity Generators are not required to file the Annual Hazardous Waste Report in accordance with LAC 33:V Chapters 1, 11, 15, 17-21, 23-29, and 31-33, 35, 37.

The "2011 ANNUAL HAZARDOUS WASTE REPORT INSTRUCTIONS AND FORMS BOOKLET  provides important information to aid in the completion of the report.  If you choose to report via hard-copy, the "2011 ANNUAL HAZARDOUS WASTE INSTRUCTIONS AND FORMS BOOKLET"  contains the forms that are needed to complete your report.

Please review pages 2 and 3 of the 2011 Annual Hazardous Waste Report Instructions and Forms Booklet to see changes to the 2011 report.

Changes were made in 2009 to the RCRA Subtitle C Site Identification Form boxes 1,4,8,10, and 12 and are still applicable.  Louisiana has not adopted the changes to Section 10D and Section 12 of the RCRA Subtitle C Site Identification Form or to the Addendum to the RCRA Subtitle C Site Identification Form.  Those sections should be left blank as they are not applicable to Louisiana reporters at this time.  All boxes on the Generation and Management Form and the Waste Received from off-site form must be completed.  A box has been added to the GM form to document Waste MinimizationThe Off-site Identification Form must be submitted as a part of the Annual Hazardous Waste Report per Federal Regulations 40 CFR 262.41(3)&(4).

The Louisiana Department of Environmental Quality (LDEQ) encourages the use of the electronic reporting software to complete your report.  Electronic reporters may use the "BRSTATE SOFTWARE" to submit the 2011 Annual Hazardous Waste Report.  The "BRSTATE SOFTWARE" was developed by the State of Florida and is offered as public domain freeware.  The electronic reporting software should be installed on a PC running WIN95/98/Me/2000/NT/XP/Windows 7/Windows Vista in order to electronically report hazardous waste management for 2011.  After installation, please consult the online instructions for help and details on how to use the program. The updated version of the software must be downloaded for completion of the 2011 Annual Hazardous Waste Report.

"CLICK HERE"  to download and install the "BRSTATE SOFTWARE" following online instructions provided to guide you through the download.  Included separately from the "BRSTATE SOFTWARE" is the "END-USER LICENSE AGREEMENT" (EULA) document that should be read prior to installation and use of the software.

Workshops provide an excellent opportunity for representatives from LDEQ to address any questions or concerns that you may have regarding completion of the Annual Hazardous Waste Reports.  Topics discussed at the workshop will include changes to the 2011 report, reporting forms, and use of the electronic reporting software.  The "WORKSHOP SCHEDULE" lists the dates and times that the workshops will be held.  The 2011 Annual Hazardous Waste Report must be received by LDEQ on or before March 1, 2012 in order to meet the regulatory reporting deadline.  The Workshop Powerpoint presentation may be of assistance in filling out your report.

Please remember that the report encompasses all of your hazardous waste activity for 2011.  Do not exclude any 2011 hazardous waste activities by submitting your report early.  Your report should not be submitted until after January 1, 2012.  The 2011 reporting year is a federal and state reporting year.  Reports should be submitted to Louisiana Department of Environmental Quality (LDEQ) where the information will be collected and compiled.
